ACEM is committed to improving outcomes for regional, rural and remote (RRR) ED patients. The Blended Supervision Pilot (BSP) Project 2022-2025 is one of several initiatives to promote this by providing high-quality ED training in expanded RRR settings and will support the implementation of the RRR Training Requirement for new trainees from 2027.
The BSP evaluated whether sites ineligible for standard FACEM Core ED accreditation could provide a six FTE month, 100 per cent ED training FACEM Trainee placement using a blended supervision model. The model is a combination of onsite clinical supervision (FACEM or RG with EM qualification), and remote training (ACEM DEMT) supervision.
In December 2025, the Council of Education and ACEM Board approved the implementation of the Blended Supervision Placement model. Preparation is underway to roll out the model for new sites in late 2026. Pilot sites can continue to recruit trainees for their sites:
- Broome Hospital (WA)
- Busselton Hospital (WA)
- Macksville District Hospital (NSW)
- Northeast Health Wangaratta (VIC)
- Warwick Hospital (QLD)
Pilot site criteria
To ensure the effective and safe delivery of the Blended Supervision model, pilot sites met specific criteria:
- Site description: Generally Level 3+ as per ACEM's S12 guideline.
- Clinical supervision: Onsite Fellow (FACEM, FACRRM/FRACGP+EM qualification) available for 50 per cent of the trainee’s rostered time, with appropriate on-call support.
- Training supervision: A local onsite supervisor (FACEM, FACRRM/FRACGP+EM qualification) and a remote Director of Emergency Medicine Training (DEMT).
- Educational support: An average of four hours per week of structured, protected education time, with up to 50 per cent of this delivered by another site.
- Assessment: Trainees are expected to complete regular workplace-based assessments (WBAs) in line with their training stage, with a recommended WBA completion rate of once per month.
- Trainee eligibility: Only one trainee per site. Must be in TS2/3 and undertaking training towards core ED or maintenance training. Trainees in Additional Training Time are not eligible.
- Placement: Maximum six-month full-time equivalent (FTE) core ED placement.
